How to Receive Your Rebate!

1. Get a contact lens fitting and/or eye exam.
2. Purchase required boxes as indicated above.
3. Complete the mail-in certificate.
4. Attach your original dated sales receipt (photocopies are not acceptable), for the qualifying purchases and a copy of your valid eye exam and/or contact lens fitting receipt.
5. Mail in the original package UPC codes as indicated above next to the product purchased, your original sales receipt indicating total purchase quantity, a copy of your original eye exam or contact lens fitting receipt and this Official Mail-In-Certificate to:

6. Keep a copy of your paperwork for your records.

Valid on purchases January 1, 2010 through July 31, 2010. Rebate submission must be postmarked by August 31, 2010. Only available while supplies of rebate forms last.
